If you would like to make a bequest to the Multiple Myeloma Research Foundation, please use the following language.

• "I give, devise, and bequeath to the Multiple Myeloma Research Foundation, a nonprofit corporation existing under the laws of the state of Connecticut, located in Norwalk, Connecticut (Federal Tax ID #06-150-4413), the sum of $__________ to be used for its general objectives and purposes."
